Patient Financial Information - Practice Philosophy

Welcome: our goal is to provide you and your family with optimum dental care. We encourage you to be involved in the treatment decisions, options, and costs. This includes understanding your treatment plan as well as out financial policies.

Patients are expected to pay for our services at the time they are rendered or make arrangements for payment as follows.

  • Cash, personal check, debit card, Visa, MasterCard, Discover or American Express. If payment is made in full at the time the treatment is performed a bookkeeping discount will be given.
  • 6,12,18, or 24 no interest monthly payments. 24,36,48, or 60 low interest monthly payments. Administered by CareCredit.
  • In Office Monthly Payments ( with approved credit, and 18% annual interest)

Dental Benefit Information

Please keep in mind that dental insurance is benefit only. Most dental insurance plans do not cover 100% of the cost of your treatment. In some cases, the treatment recommended is well over what benefits you have per year to use. We will try to phase treatment to best utilize your yearly benefits, but sometimes the treatment cannot wait, and it would be worse and cost more if we did wait.

As a courtesy to our insured patients, we submit claims to your insurance company free of charge. If you insurance has not paid within 45 days of treatment you will need to make full payment to this office and be reimbursed when your insurance company pays.

You are also responsible to pursue payment from the insurance company after 45 day. All current documentation will be provided by mail in order to assist your inquires.

Please keep in mind that the dental insurance contract is an agreement between the patient and the insurance company: therefore, the patient is responsible for the bill regardless of the insurance coverage.

Please be familiar with the coverage and deductible on your insurance plan(s). To help you better understand your dental benefits, read your plan description and call your employer, Personnel dept. or Insurance Company regarding any questions you may have.
